01

The program

The Cairava heart-failure transition program sends text messages to patients who have been discharged from a participating hospital after a heart-failure admission, and to the caregivers those patients name. Cairava runs the program on behalf of the hospital. Your care team remains your care team.

Enrollment is voluntary. You may decline, and declining does not change the care you receive from the hospital.

02

What you receive

  • A daily check-in asking how you are feeling and, on most days, for your weight.
  • Reminders about the medicines on your discharge list, at times you choose.
  • Reminders about follow-up appointments, usually two days before and again on the day.
  • Replies and questions from the people on your care team.
  • Short education messages about living with heart failure, about twice a week.

03

How often

Message frequency varies depending on where you are in your 30-day program. You will receive no more than 3 program-initiated messages on any day. Replies from your care team to something you sent are not counted against that limit.

The program does not send routine messages between 8pm and 9am in your local time.

04

Cost

Message and data rates may apply. Cairava does not charge you for the messages; your mobile carrier may.

05

Getting help

Reply HELP to any message and you will receive a reply telling you what the program is and how to reach someone. You can also email help@cairava.com.

Cairava is not for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency department.

06

Stopping messages

Reply STOP to any message to stop all program messages. It is free, and it takes effect immediately. You will receive one confirmation message and then nothing further.

One thing you should know before you do it: stopping messages during the weeks after a heart-failure admission is treated as something your care team should hear about, not as a preference setting. When you reply STOP, the program stops texting you and tells your care team that it has. Someone from the hospital may call you. You are free to stop the messages and free to decline the call.

To start again, reply START, or tell anyone on your care team.

07

How we get your permission

Someone at the hospital asks you to join the program, usually around the time you are discharged, and records your written agreement along with the date, the time, and how you gave it. We do not add anyone from a purchased list, and we do not text a number that was not given to us for this purpose.

If you name a caregiver who should also receive messages, we ask for their agreement separately.

08

Carriers and delivery

Carriers are not liable for delayed or undelivered messages.

The program supports mobile numbers in the United States. Messages may not reach landlines or some prepaid services.

09

Your number

Your mobile number and the fact that you opted in are used only to run this program. They are never sold, and they are never shared with third parties or affiliates for marketing purposes.
